Amaç: Kliniğimizde pars plana vitrektomi (PPV) uygulanan hastaların, cerrahi sonrası erken ve geç dönem göz içi basıncı (GİB) artış nedenleri, sıklığı, cerrahi yöntemlerle olan ilişkisi ve uygulanan tedavi yöntemlerini araştırmaktır. Gereç ve Yöntemler: Ocak 2013-Ocak 2016 tarihleri arasında, PPV uygulanan 107 olgunun GİB artışı saptanan 46'sında tedavi yöntemleri ve basınç artış mekanizmaları retrospektif olarak incelendi. Bulgular: Bu çalışmada 29-86 yaş aralığında, (60,44 ± 11,04) 38 (%35,5) kadın, 69 (%64,5) erkekden oluşan 107 hastanın 107 gözü değerlendirildi. PPV sonrasında sekonder glokom tanısı alan 23 gözden 22 (%95,7) göze endotamponad olarak silikon yağı ve 1 (%4,3) göze hava verildiği saptandı. Glokom gelişimi ile silikon yağı arasındaki istatistiksel ilişki anlamlı bulunmasına rağmen (p=0,008) uzun etkili gaz tamponad kullanımı ile arasındaki ilişki anlamsız bulunmuştur (p>0,05). GİB medikal tedavi ile 44 (%95,6) hastada 21 mm/Hg'nın altında düştü. Medikal tedaviye yanıt alınamayan 2 (%4,3) hastaya cerrahi tedavi uygulandı. Sonuç: PPV sonrası GİB artışı sık rastlanan bir komplikasyondur. Bunun için hastalar postoperatif her dönemde yakından takip edilmelidir.

Objective: The aim of this study was to investigate the causes of early and late postoperative intraocular pressure (IOP) increases, frequency, the relationship with surgical methods, and treatment modalities in patients who underwent pars plana vitrectomy (PPV) in our clinic. Material and Methods: The files of 107 cases with PPV between January 2013 and January 2016 were retrospectively analyzed. 46 of the 107 eyes showed an increase in IOP. The mechanisms and treatment modalities of elaveted IOP were evaluated. Results: In this study, 107 eyes of 107 patients 38 (35.5%) female and 69 (64.5%) male between the ages of 29-86 (60.44±11.04) years were evaluated. 22 of 23 (95.7%) eyes with secondary glaucoma diagnosis were treated with silicone oil as endotamponad and 1 (4.3%) eye was treated with air during PPV. Although the statistical relationship between the development of glaucoma and to use silicone oil was found to be significant (p=0.008), there was no significant relationship with to use long-acting gas tamponades (p>0.05). IOP was reduced by medical theraphy 21 mm/Hg or less in 44 (95.6%) patients. Surgical treatment was performed in 2 (4.3%) patients who unresponsive to medical treatment. Conclusion: IOP elevation after PPV is a common complication, so patients should be closely monitored for IOP values at each postoperative period.
