ÖZET
Amaç: Atopik dermatit (AD)'te insanlara benzer şekilde köpekler için de öne sürülen hipotezlerden biri, sızıntılı bağırsak modeline eşlik eden intestinal permeabilite artışı ve bağırsak mikrobiyotasının bozulması olsa da konuya ilişkin deride korneometrik analizlere dair bilinmeyenler mevcuttur. Bu araştırmada, kaşıntının en önemli nedenlerinden biri olan AD'de; 1) Korneometrik analizlerin ortaya konularak tanısal birer biyobelirteç olarak kullanılıp kullanılamayacağının belirlenmesi, 2) Makroskobik görünüm ile korneometrik analizler doğrultusunda lezyon haritasının oluşturularak ileride muhtemel sağaltım protokollerinde değişim sağlayıp sağlayamayacağının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: Araştırma grubumuz, toplamda 65 AD'li köpek (farklı yaş, ırk ve cinsiyetten) ile 10 sağlıklı kontrolden oluşturuldu. Hasta gruba (AD) dâhil edilme kriterleri belirlenirken önceden herhangi bir sağaltım uygulaması yapılmamış, Favrot kriterleri ve atopi ile uyumlu klinik bulgular gösteren, alerjen-spesifik IgE düzeyinde artış şekillenmiş olgular ve CADESI-04 skorlamaları baz alındı. Kontrol grubunda sağlıklı köpekler yer aldı. Korneometrik analizler Callegari Soft Plus cihazı ile yapıldı. Bulgular: CADESI-04 skorlamaları dâhilinde 12 olgu hafif, 24 olgu orta, 29 olgu ise şiddetli AD olarak sınıflandırıldı. Sekonder komplikasyon görülme oranı sırasıyla hastalık şiddetine göre 2/11, 8/24 ve 10/29 olarak belirlendi. Epidermal pH (4,1-5,2, 3,9-5,6 ve 3,3-5,8) ve hidrasyon (17-47, 5-40 ile 0-35) değerlerine sırasıyla gruplarda belirgin farklılıklar mevcuttu. Her olguya ait makroskopik lezyon haritası çıkartıldı. Kaşıntı skorları yine hastalık aktivitesi ile doğru orantılı olarak hafif, orta ve şiddetli olgularda sırasıyla 1-5, 1-8 ve 4-10 arasında farklılık gösterdi. Sonuç: Yapılan çalışma ile detaylı demografik bilgiler ve vaka haritaları çıkartılmasının AD hastalığının tanı ve tedavisine klinik yaklaşımda faydalı olduğu görüldü.

ABSTRACT
Objective: Given atopic dermatitis (AD), similar to human being in dogs, one of the hypotheses proposed is increased intestinal permeability along with leaky gut and the deterioration of gut microbiota, altough unknown parts appear regarding epidermal corneometric analysis. In the present study the aim was to make interpretation 1) Through evaluation of corneometric analysis usage as a diagnostic biomarker, 2) Performing lesional mapping by use of macroscopical appearence within corneometric analysis for further detection of possible alterations within treatment protocoles. Material and Methods: A total of 65 dogs with AD (different age, race and sex) and 10 healthy controls were included in the study. The inclusion criteria for the patient group (AD) were based on the patients who did not undergo any treatment before, showed clinical findings consistent with Favrot criteria and atopy, an increase in allergen-specific IgE levels and CADESI-04 scores. The other group consisted of healthy and control dogs. Corneometric analyzes were performed with Callegari Soft Plus device. Results: According to CADESI-04 scores, 12 cases were classified as mild, 24 cases as moderate and 29 cases as severe AD. The incidence rate of secondary complications was determined as 2/11, 8/24 and 10/29 according to the severity of the disease. Significant differences were present in groups with epidermal pH (4.1-5.2, 3.9-5.6 and 3.3-5.8) and hydration (17-47, 5-40 and 0-35) respectively. Macroscopic lesion map of each case was taken and presented in the images in the article. Pruritus scores also ranged from 1-5, 1-8 and 4-10, respectively, in mild, moderate and severe cases, directly proportional to disease activity. Conclusion: It was thought that prepared of detailed demographic information and case maps is usefull to clinical approach to AD disease.

8. LAW AND AUTHORISATION TO FOLLOW

Turkish Law will be applied in practicing, interpreting the hereby "Terms of Use" and managing the emerging legal relationships within this "Terms of Use" in case of finding element of foreignness, except for the rules of Turkish conflict of laws. Ankara Courts and Enforcement Offices are entitled in any controversy happened or may happen due to hereby contract.

9. CLOSING AND AGREEMENT

Hereby "Terms of Use" come into force when announced in the "SITE" by "Turkiye Klinikleri". The users are regarded to agree to hereby contract terms by using the "SITE". "Turkiye Klinikleri" may change the contract terms and the changes will be come into force by specifying the version number and the date of change on time it is published in the "SITE".
