Amaç: Bu çalışmanın amacı, konik ışınlı bilgisayarlı tomografi (KIBT) görüntülerinde farklı mandibular kondil morfolojilerine sahip hastaları sınıflandırarak kondil tiplerini fraktal analiz (FA) ile değerlendirmektir. Gereç ve Yöntemler: Çalışmaya ağız, diş ve çene radyolojisi ana bilim dalı arşivinde kayıtlı, KIBT görüntülerinde temporomandibular eklemin net olarak izlendiği 50 hasta dâhil edildi. Her hasta için sağ ve sol kondiler morfoloji, koronalde mesio-distal olarak en geniş izlendiği kesit incelenerek konveks, yuvarlak, düz ve açılı olarak sınıflandırıldı. FA, Image J programı kullanılarak hesaplandı. Elde edilen veriler SPSS 20 programına aktarılarak istatistiksel olarak karşılaştırıldı. Bulgular: Çalışmada, yaş aralığı 18-60 olan 22 kadın ve 28 erkek hastanın mandibular kondil morfolojileri değerlendirildi. Kondiler morfolojiler vakaların %36'sında yuvarlak, %29'unda dışbükey, %27'sinde düz, %8'inde açılı olarak tespit edildi. Kadın hastalarda en sık yuvarlak (%40,9) kondil tipi, erkek hastalarda en sık yuvarlak ve dışbükey (%32,1) kondil tipi izlendi. Mandibular kondillerin FA değerleri 1,05-1,47 değerleri arasındaydı. FA değeri, mandibular kondil tipleri arasında istatistiksel olarak anlamlı farklılık gösterdi (p<0,05). Düz kondil tipi en düşük, açılı ve dışbükey kondil tipi en yüksek değere sahipti. FA değeri, yaş grupları ve cinsiyetler arasında anlamlı bir farklılık göstermedi. Sonuç: Yuvarlak, dışbükey ve açılı kondil tiplerinin fraktal değerleri daha yüksek iken düz kondil tipinde fraktal değerlerin daha düşük olduğu tespit edilmiştir. Düz kondil tipinde fraktal değerlerin düşük olması, bu tipteki hastalarda kemik kalitesinin azalabileceğini ve olası kemik patolojilerine karşı dikkatli olunması gerektiğini göstermektedir.

Objective: The aim of this study was to classify patients with different mandibular condyle morphologies on cone beam computed tomography (CBCT) images and to evaluate condyle types using fractal analysis (FA). Material and Methods: The study included 50 patients registered in the archive of the department of oral, dental and maxillofacial radiology, in whom the temporomandibular joint was clearly visualised on CBCT images. For each patient, the right and left condylar morphology was classified as convex, rounded, flat and angled by examining the mesio-distally widest section in the coronal view. FA was calculated using Image J programme. The obtained data were transferred to SPSS 20 programme and compared statistically. Results: In the study, mandibular condyle morphologies of 22 female and 28 male patients with an age range of 18-60 years were evaluated. Condylar morphologies were round in 36%, convex in 29%, flat in 27%, and angled in 8% of the cases. The most common condyle type in female patients was round (40.9%), and the most common condyle type in male patients was round and convex (32.1%). The FA values of the mandibular condyles were between 1.05-1.47. FA values showed a statistically significant difference between the mandibular condyle types (p<0.05). Straight condyle type had the lowest value, angled and convex condyle type had the highest value. FA value did not show a significant difference between age groups and genders. Conclusion: Round, convex and angled condyle types had higher fractal values, while flat condyle type had lower fractal values. The low fractal values in the flat condyle type indicate that bone quality may decrease in patients with this type and caution should be exercised against possible bone pathologies.
