ÖZET
Hemodiyaliz, tedavi edici özelliğinin yanı sıra hastada farklı problemlerin ortaya çıkmasına da neden olmaktadır. Bakımın görünürlüğünü artırmak, kaliteli bakım sunabilmek ve ortak bir dil kullanmak için sınıflama sistemlerinin kullanılması gerekmektedir. Olguda Fonksiyonel Sağlık Örüntüleri Modeli (FSÖ), Kuzey Amerika Hemşirelik Tanıları Birliği (NANDA), Hemşirelik Girişimleri Sınıflandırması (NIC), Hemşirelik Sonuçları Sınıflandırması (NOC) ile hemodiyaliz tedavisi uygulanan bir hasta değerlendirilmiştir. Bu olgu sunumunda amaç, hemodiyaliz tedavisi alan bir hastayı değerlendirmek, NANDAI Taksonomi II ile hemşirelik tanıları, NIC girişimleri ve NOC sonuçları doğrultusunda verilen hasta bakımını sunmaktır. Olgu bir üniversite hastanesinin 3. düzey dâhiliye yoğun bakım ünitesinde araştırmacı tarafından 9 gün izlenmiştir. Olguda verilen bakımın değerlendirmesi 1, 4 ve 7. günlerde bakım veren araştırmacı tarafından NOC sistemine göre yapılmıştır. Hastanın bakımı NANDA Taksonomi II'de yer alan sağlığı algılama, beslenme-metabolik, aktivite-egzersiz, uyku-dinlenme alanlarından 8 hemşirelik tanısı, NOC'da yer alan 8 hemşirelik bakım sonucu ve NIC'de yer alan 13 hemşirelik girişimi ve girişimlerin altında yer alan hastaya özgü seçilmiş 133 aktiviteyi içermiştir. Verilerin FSÖ modeli doğrultusunda toplanması hastanın kapsamlı bir şekilde ele alınmasını sağlamıştır. Sonuç olarak FSÖ modeli, NANDA, NIC ve NOC sistemlerinin kullanılması ile hemodiyaliz tedavisi uygulanan hastanın gereksinimlerinin daha iyi saptandığı düşünülmektedir. Bakım sonrası en çok değişim sağlanan NOC sonuçları 22 puanlık fark ile ''0504. Renal fonksiyon'' ve ''0004. Uyku'' olmuştur.

ABSTRACT
Hemodialysis, besides its therapeutic features, may cause deriving of different problems for patient. Use of classification systems is necessary in order to increase the visibility of care, to provide qualified care and to use a common language. In this case a patient who underwent hemodialysis treatment with the Functional Health Patterns Model (FHP), North American Nursing Diagnosis Association (NANDA), Nursing Intervention Classification (NIC) and Nursing Outcomes Classification (NOC) were evaluated. The aim of this case report is to evaluate a patient receiving hemodialysis treatment and to present the patient care given by NANDA-I Taxonomy II according to nursing diagnosis, NIC interventions and NOC outcomes. The case was monitored by researcher in a third level internal medicine intensive care unit of a university hospital for a period of 9 days. The assessment of the care given in the case was performed by the researcher on days 1, 4 and 7 according to the NOC system. 8 nursing diagnosis from the areas of nutrition-metabolic, activityexercise and sleep-rest included in NANDA Taxonomy II, 8 nursing care outcomes included in NOC and 13 nursing interventions included in NIC and also selected 133 activities specific to the patient underwent interventions are included. The collection of data in line with the FHP model has provided a comprehensive overview of the patient. Consequently, the needs of the patient experiencing hemodialysis treatment with the utilization of Functional health patterns model, NANDA, NIC and NOC systems were better determined. The NOC outcome with the most significant difference was ''0504. Renal function'' and '0004. Sleep' with 22 points.

8. LAW AND AUTHORISATION TO FOLLOW

Turkish Law will be applied in practicing, interpreting the hereby "Terms of Use" and managing the emerging legal relationships within this "Terms of Use" in case of finding element of foreignness, except for the rules of Turkish conflict of laws. Ankara Courts and Enforcement Offices are entitled in any controversy happened or may happen due to hereby contract.

9. CLOSING AND AGREEMENT
