Amaç: Artrosentez, özellikle ağız açmada kısıtlılık ve ağrı ile seyreden temporomandibular bozuklukların (TMB) tedavisinde etkili bir yöntemdir. Bu çalışmanın amacı farklı yaş gruplarında artrosentezin tedavi etkinliğinin değerlendirilmesidir. Gereç ve Yöntemler: Bu retrospektif çalışmada, "Diagnostic Criteria of Temporomandibular Disorders (DC/TMD)" sınıflamasına göre, red¨¹ksiyonsuz disk deplasmanı tanısı konulan ve kliniğimizde artrosentez tedavisi olan 109 hastanın tedavi kayıtları kullanılmıştır. Yaş, cinsiyet, operasyon öncesi ağrı ve maksimum ağız açıklığı (MAA) değerleri işlem öncesi veriler olarak kaydedilmiştir. Ağrı değeri için viz¨¹el analog skala (VAS) kullanıldı. Artrosentez tedavisinin başarısı 1. ay takibinde 3 farklı yaş grubu için değerlendirildi: Grup 1 (29 yaş ve altı), Grup 2 (30-49 yaş) ve Grup 3 (50 yaş ve ¨¹zeri). Bulgular: Çalışmaya, yaş ortalaması 31,64 olan 97 (%89) kadın, 12 (%11) erkek dâhil edildi. İşlem öncesi VAS ve MAA değerlerinde anlamlı bir fark bulunmamıştır (p>0,05). Artrosentezden sonra birinci ay takipte işlem öncesine göre VAS değerlerinde azalma ve MAA seviyesinde artış gör¨¹lm¨¹şt¨¹r (p<0,05). İşlem öncesi ve sonrası VAS değerleri arasındaki fark 50 yaş ¨¹zeri grupta diğer yaş gruplarına göre anlamlı olarak daha fazla bulunmuştur (p<0,05). MAA açısından gruplar arasında bir fark bulunmamıştır (p>0,05). Sonuç: Bu çalışmanın sonuçlarına göre artrosentez, TME red¨¹ksiyonsuz disk deplasmanlarının tedavisinde etkili bir yöntem olarak bulunmuştur. Artrosentezin ağrının giderilmesi ¨¹zerindeki etkisi 50 yaş ve ¨¹zeri hastalarda genç ve orta yaşlı hastalara göre daha fazladır.

Objective: Arthrocentesis is an effective treatment modality for temporomandibular disorders (TMD), especially in patients who suffer from pain and limited mouth opening. The purpose of this study is to investigate the treatment effectiveness of arthrocentesis procedure in different age groups of patients with TMD. Material and Methods: The records of 109 patients diagnosed as disc displacement without reduction according to "Diagnostic Criteria of Temporomandibular Disorders (DC/TMD)", and treated with arthrocentesis in our clinic were selected for this retrospective study. Age, gender, pain intensity, maximum mouth opening (MMO) was recorded as preoperative variables. Pain intensity was evaluated with visual anlogue scale (VAS) treatment effectiveness in the first month follow up was compared among 3 age groups: Group 1 (aged <29 years), Group 2 (aged 30-49 years) and Group 3 (¡İ50 years). Results: The analysis included 109 patients 97 (%89) female, 12 (%11) male with a mean age of 31,64 years. There was no significant difference between the preoperative VAS and MMO scores (p>0.05). In postoperative first month follow up, VAS scores decreased and MMO increased significantly when compared to preoperative scores (p<0.05). When preoperative and post-operative results were compared, significant increase in VAS score difference was only seen in patient older than 50 years (p<0.05). No difference was found in MMO between the age groups (p>0.05). Conclusion: Our findings suggested that arthrocentesis is an effective treatment protocol in patients with disc displacement without reduction. The effect of arthrocentesis on pain relief is more effective in patients older than 50 years than younger and middle aged patients.
