Amaç: Bu çalışma, kronik bel ağrısı (KBA) tanılı hastalarda, transversus abdominis (TA), multifidus (M) kas aktivitesi ve dinamik dengeyi sağlıklı kontrollerle karşılaştırmak ve TA ve M kas aktivitesi ile dinamik denge arasındaki ilişkiyi incelemek amacıyla planlandı. Gereç ve Yöntemler: Çalışmaya, KBA tanılı 60 hasta ve 60 sağlıklı kontrol grubu dâhil edildi. Katılımcıların TA ve M kaslarının aktiviteleri, basınçlı biofeedback cihazı (Chatanooga Stabilizer Pressure Biofeedback, Avustralya, 2005) ile dinamik dengeleri ise Y denge testi ile anterior, posterolateral, posteromediyal olmak üzere 3 yönde değerlendirildi. Bulgular: Hastaların, yaş ortalamaları 42,88±11,28 yıl, sağlıklı kontrollerin yaş ortalamaları 38,33±12,20 yıl olarak bulundu. Hastalar ve sağlıklı kontroller arasında TA ve M kas aktivitesi ölçüm sonuçları bakımından fark bulundu (p=0,001). Hastaların Y denge testi anterior, posterolateral ve posteromediyal sonuçlarının sağlıklı kontrollere göre anlamlı şekilde düşük olduğu görüldü (p=0,001). Hastaların TA kas aktivitesi sonuçları ile Y denge testinin posteromediyal yönü ve posterolateral yönü arasında anlamlı ilişki bulundu (p=0,001). Sonuç: Çalışma sonucunda, KBA'lı hastalarda azalmış TA ve M kas aktivitesi ve dinamik dengelerinin olumsuz etkilenimi gösterildi. TA kas aktivitesinin azalmasının dinamik dengenin posterior paramatrelerini olumsuz etkilediği görüldü. Bu sonuçlar, KBA'lı hastalara uygulanan mevcut fizyoterapi ve rehabilitasyon programlarında, lumbal bölge derin kaslarının gelişimini hedef alan stabilizasyon eğitimlerine yer verilmesinin önemli olduğunu vurgular niteliktedir.

Objective: The purpose of this study was to compare the dynamic balance of Transverse Abdominis (TrA), Multifidus (M) muscle activity with healthy controls and to investigate the relationship between TrA and M muscle activity and dynamic balance in patients with a diagnosis of chronic low back pain (CLBP). Material and Methods: 60 patients with CLBP and 60 healthy controls were included in the study. The activities of TrA and M muscles of the participants were evaluated with pressurized biofeedback device (Chatanooga Stabilizer Pressure Biofeedback, Avustralya, 2005) and dynamic balances were evaluated by Y Balance Test in three directions: anterior, posterolateral and posteromedial. Results: The mean age of the patients was 42.88±11.28 years and the mean age of the healthy controls was 38.33±12.20 years. There was a difference between patients and healthy controls in terms of TrA and M muscle activity measurement results (p=0.01). Anterior, posterolateral and posteromedial results of the Y Balance Test were found to be significantly lower than healthy controls. (p=0.01). There was a significant correlation between TrA muscle activity results and posteromedial and posterolateral aspects of Y Balance Test (p=0.01). Conclusion: As a result of this study, decreased TrA and M muscle activity and dynamic balance were negatively affected in patients with CLBP. Decrease in TrA muscle activity negatively affected posterior parameters of dynamic balance. These results emphasize that it is important to include stabilization trainings aimed at the development of deep muscles of the lumbar region in current physiotherapy and rehabilitation programs applied to patients with CLBP.
