Amaç: Kedilerde de yaygın olarak gözlemlenen astımın patogenezinde havayolları yangısı, havayollarının aşırı duyarlılığı ve geri dönüşümlü tıkanması sorumludur. Bu kohort tipi prospektif araştırmada, yazarlar ağzı açık soluyan ve astım tanısı konulan kedilerde olası bir biyobelirteç olarak hiperamonyakemi ve ölçülemese de muhtemel hipoarjinineminin (sağaltımdan teşhise) olağan suçlular olup olamayacağı hipotezini ortaya koymayı amaçlamışdır. Gereç ve Yöntemler: Çalışma kapsamına dâhil edilen kedilerde astım teşhisi; a) klinik bulgular, b) torasik radyografi, c) ekokardiyografik muayene ve d) Polycheck in vitro alerjen tayini ile gerçekleştirildi. Sağaltım oral yolla uygulanan L-arjinin ekstraktı ile immünonütrisyon, ilişkili nutrasötikler, probiyotikler (bireysel olarak reçetlendirilen), diyet kısıtlamaprovokasyon testini (çalışma periyodu boyunca glutensiz diyet önerildi) içermekteydi. Bulgular: Sağaltım başarısı ve prognoz açısından 0. gün (herhangi bir sağaltım uygulanmadan) ve 30. gün (sağaltım sonrası) Hayvan Travma ve Triyaj Skorlaması değerlendirildi. Hayvan Travma ve Triyaj Skorlamasına ilişkin ortanca (median) değerleri perfüzyon, kardiyak, respiratuar ve nörolojik değerlendirmelere göre sağaltım öncesi sırası ile 2, 1, 2 ve 1,5 olarak saptanırken, sağaltım sonrası her 4 sistem muayenesine yönelik ortanca değerler 0 olarak klinik iyileşmeye eş zamanlı saptandı. Sonuç: Uzun dönem monitorize edilen kedilerde arjinin ile zenginleştirilen probiyoterapinin, astım sağaltımında yeni bir dönemi açabileceği öne sürülebilinir.

Objective: As asthma is a widely observed disease in cats, its pathogenesis involve airway inflammation, airway hyperresponsiveness and reversible airway obstruction. In the present cohort type prospective study the authors hypothesized that hyperammonia and altough not analyzed, possible hypoargininemia (from treatment to diagnosis) may be the usual culprit to those of panting cats with a diagnosis of feline asthma. Material and Methods: To those of cats with a diagnosis of asthma, diagnosis was based on a) clinical signs, b) thoracic radiography, c) echocardiography and d) Polycheck in vitro allergen analysis. Treatment involved immunonutrition with L-arginine extract given orally, relevant nutraceuticals, probiotics (individually prescribed), a dietary restriction-provocation test (involving gluten free diet at study period). Success of therapy and prognosis was monitorized by Animal Trauma and Triage Scoring System before (day 0 prior to any treament) and after (day 30) treatment. Results: Regarding Animal Trauma and Triage Scoring System median values for perfusion, cardiac, respiratory and neurological interpretation prior to treatment respectively detected as 2, 1, 2 and 1.5, deemed whole median values as 0 for 4 system evaluation, in parallel line with clinical recovery. Conclusion: To those of cats monitorized for long term, should open a new era for the treament of asthma with arginine enriched probiotherapy.
