Hiperfizyolojik miktarda trombosit ve büyüme faktörü içeren trombositten zengin plazma (TZP), antikoagülanlı tüpe alınmış tam kanın santrifüj edilmesiyle elde edilir. TZP, hastanın kendi kanından elde edildiği için çabuk ve güvenle uygulanabilen rejeneratif tıp yöntemlerinden biridir. TZP, açık veya kapalı teknikle hazırlanır. Aferez, Buffy Coat ve TZP üretim teknikleri gibi farklı teknikler kullanılarak elde edilir. Temelde yumuşak dönüş sonrası sert dönüş olarak adlandırılan, antikoagülanlı tam kanın ardışık 2 santrifüjüyle elde edilebilmektedir. Piyasada pek çok farklı hazır TZP kiti bulunmaktadır. TZP hazırlanırken hematokrit, sıcaklık, pH, aktivasyon yöntemi, kan alma süresi, kan alınırken kullanılan kanülün çapı, kullanılan protokol, kan alma yeri, alıcının yaşı ve dokunun durumu gibi TZP'nin etkinliğini etkileyen faktörlere dikkat edilmelidir. TZP dondurulup kurutularak 8 haftaya kadar muhafaza edilebilir ve bu sürede trombosit işlevi, sitokin konsantrasyonu ve işlevselliği korunur. TZP, maksillofasiyal cerrahi, plastik ve rekonstrüktif cerrahi, algoloji, kas-iskelet sistemi hastalıkları (kemik defektleri, kas-ligamet hasarları, tendinopatiler ve osteoartrit gibi), göz hastalıkları (kornea ülserleri ve kuru göz sendromu gibi), infertilite ve daha pek çok alanda kullanılabilir. TZP'nin ucuz ve kolay elde edilebilme, inflamasyonu sınırlandırma ve iyileşmeyi hızlandırma gibi avantajlarının yanında yaygın olmayan, hafif yan etkileri de vardır. Bu derlemede, TZP'nin hazırlanışı, etkinliğini etkileyen faktörler, muhafazası, endikasyonları, etki mekanizması, avantajları, dezavantajları ve yan etkileri hakkında bilgiler sunulmaktadır.

Platelet-rich plasma (PRP) containing hyperphysiological amounts of platelets and growth factors is obtained by centrifuging whole blood in an anticoagulant tube. PRP's one of the regenerative medicine methods that can be applied safely and quickly because it's obtained from the patient's own blood. PRP is prepared with open or close technique. It's obtained using different techniques such as Apheresis, Buffy Coat and PRP production techniques. Basically, it can be obtained by 2 consecutive centrifuges of anticoagulant whole blood, which is called soft spin after hard spin. There are many different ready-made kits on the market. While preparing PRP, attention should be paid to factors affecting the effectiveness of PRP, such as hematocrit, temperature, pH, activation method, blood collection time, diameter of the cannula used for blood collection, protocol used, blood collection spot, recipient's age, and tissue condition. By freeze-drying PRP, platelet function, cytokine concentration and functionality are preserved and can be preserved for 8 weeks. PRP, maxillofacial surgery, plastic and reconstructive surgery, algology, musculoskeletal system diseases (such as bone defects, muscleligaments injuries, tendinopathy and osteoartritis), ophthalmological diseases (such as corneal ulcers, dry eye syndrome), infertility and can be used in many other fields. In addition to the advantages of PRP such as limiting inflammation, accelerating healing, cheap and easy to obtain, it also has uncommon, mild side effects. In this review, information about the preparation of PRP, factors affecting its effectiveness, preservation, indications and mechanism of action, advantages, disadvantages and side effects are presented.
