ÖZET
Son yıllarda, soğuk lipoliz, radyofrekans, düşük seviye lazer ve yüksek yoğunluklu odaklanmış ultrason gibi yağ dokusunun invaziv olmayan redüksiyonu için çeşitli yöntemler mevcuttur. Her bir teknoloji, hedeflenen adipositlerin apoptoz veya nekrozuna neden olmak için farklı bir etki mekanizması kullanır. Bu teknolojiler arasında soğuk lipoliz en uzun süredir piyasada bulunmakla kalmamış, aynı zamanda in vitro ve hayvan modelleri ile insanlarda randomize kontrollü klinik deneyler de dahil olmak üzere en iyi araştırılmıştır. Soğuk lipolizin ardındaki ilke, adipositlerin diğer deri hücrelerine göre soğutmaya daha duyarlı olduğu prensibidir. Soğuk derecelerin uygun şekilde uygulanması, adipositlerin apoptozunu ve inflamatuar bir yanıt ile makrofajlar tarafndan yok edilmesini sağlar. Klinik çalışmalarda, soğuk lipolizin, bir tedaviden sonra tedavi alanındaki subkutanöz yağda, %25'e varan düşmeler olduğu gösterilmiştir. Tedavi gören kişilerin % 86'sında iyileşme görüldüğü saptanan bu teknikteki %73 hasta memnuniyeti, noninvazif lipoliz için kullanılan diğer teknolojilere göre daha yüksektir. Soğuk lipolizin vücut şekillendirmesi için güvenli ve az rahatsızlık veren bir yöntem olduğu kanıtlanmıştır. Beklenen yan etkiler geçici eritem, morarma ve geçici uyuşma, yanma hissi olup tedaviden sonra genellikle 14 gün içerisinde çözülür. Hastalarda en sık görülen şikayet (%0.1 oranında) ve prosedür sonrası 2 hafta içinde müdahale olmadan çözülen ağrıdır. Noninvaziv vücut şekillendirme için altın standart olarak kabul edilmiş bir prosedür olmamasına rağmen, soğuk lipoliz hem hasta memnuniyet oranı açısından hem güvenli hem de verimli olduğu düşünülmektedir.

ABSTRACT
A number of treatment modalities have become available for the noninvasive reduction of adipose tissue, including cryolipolysis, radiofrequency, low-level laser, and high- intensity focused ultrasound,in recent years, Each of them uses a different mechanism to cause apoptosis and subsequent necrosis of the adipocytes. Among these technologies, cryolipolysis ( cold lipolysis) has not only been commercially available for the longest time, but has also been best researched including in vitro and animal models and randomized controlled clinical trials in humans. The principle behind cryolipolysis exploits the premise that adipocytes are more susceptible to cooling than other skin cells. The precise application of cold temperatures triggers apoptosis of the adipocytes, which invokes an inflammatory response and leads to slow digestion by surround ing macrophages. In clinical studies, cryolipolysis was shown to reduce subcutaneous fat at the treatment site by up to 25% after one treatment. Improvements were seen in 86% of treated subjects. At 73%, the patient satisfaction rate is higher than with other technologies used for noninvasive lipolysis. Cryolipolysis has been proven to be a very safe method for body contouring, and is accomplished with only minimal discomfort. Expected side effects are temporary erythema, bruising, and transient numbness that usually resolve within 14 days after treatment. With a prevalence of 0.1%, the most common complaint is late-onset pain, occurring 2 weeks post-procedure, which resolves without intervention. Although no procedure has been accepted as the gold standard for noninvasive body contouring as yet, cryolipolysis is considered to be both safe and efficient with a high patient satisfaction rate.
