Postmenopozal Osteoporoz Tanısında Deoksipiridinolin'in Değeri

ÖZET
Osteoporoz; kemik kitlesinde azalma ve mikroskopik yapısında bozulma sonucunda kemik frajilitesinde artma ve kırıkların kolay ortaya çıkmasıyla karakterize sistemik bir iskelet hastalığıdır. Kemik yapım ve yıkımının duyarlı ve özgül belirleyicilerinin son birkaç yılda gelişmesiyle kemik turnoverinin noninvazif ölçümünde gelişmeler olmuştur. Kollajen piridinyum çapraz bağlarının, kollajen yıkımını gösteren iyi bir idrar belirteci olduğunu ispatlayan çalışmalar yapılmıştır. Sosyal Sigortalar Kurumu Ankara Eğitim Hastanesinde hasta ve sağlıklı gönüllülerden alınan 68 idrar örneği otomatize kemilüminesan immünassay yöntem kullanılarak değerlendirildi. Bu hastalar kemik dansitometrilerine göre 3 grupta sınıflandırıldı; (Grup 1) postmenopozal osteoporotik 22 hasta, (Grup 2) postmenopozal normal 26 olgu ve (Grup 3) premenopozal normal 20 olgu incelendi. İdrar deoksipiridinolin değerleri yüksek olan 20 hasta bir bifosfonat derivesi olan Alendronate ile 4 ay süreyle tedavi edildi. İdrar Dpd/Cre değerlerini postmenopozal osteoporotik grupta, postmenopozal normal ve premenopozal normal gruba göre daha yüksek bulduk (p<0.05). Postmenopozal osteoporotik grupta dansitometri ve Dpd/Cre değerleri arasında anlamlı bir ilişki bulamadık (p> 0.1). Serum Ca+2 ve ALP değerleri postmenopozal osteoporotik grupta, postmenopozal normal ve premenopozal normal gruba göre daha yüksekti (p<0.05). Serum ve idrar Pi düzeylerinde gruplar arasında anlamlı bir ilişki bulamadık (p>0.05). Alendronat tedavisi verilen postmeno-pozal osteoporozlu hastaların tedavi öncesi ve sonrası Dpd/Cre değerleri arasında anlamlı bir düşme saptadık (p<0.05). Sonuç olarak; yaptığımız bu çalışma kollajen yıkım ürünü olan idrar deoksipiridinyumun osteoporozun erken dönemde tanısında ve tedavi takibinde iyi bir belirteç olduğunu göstermiştir.

ABSTRACT
Osteoporosis is a systemic skeletal disease characterized by low bone mass and microarchitectural deterioration with a consequent increase in bone fragility and susceptibility to fracture. The noninvasive assessment of bone turnover has markedly improved in the past few years with the development of sensitive and spesific markers of bone formation and bone resorption. Several studies in recent years have shown that the pyridinium crosslinks of collagen (especially deoxypyridinoline) provide good urinary markers of collagen degradation, primarily reflecting bone resorption. A total of 68 urine samples from patients and healthy volunteers were analysed by automated chemiluminesence immunassay system in SSK Ankara Hospital. They were classified according to their bone densitometric values; Postmenopausal osteoporotic group 1 (n=22); postmenopausal normal group 2 (n=26); premenopausal normal group 3 (n=20). Twenty patients who have high urinary Dpd were treated with alendronate for 4 months. We found that the urinary Dpd/Cre values in postmenopausal osteoporotic group were higher than postmenopausal and premenopausal group had (p<0.05). We could not observe any significant relationship between bone densitometry and Dpd/Cre values in postmenopausal osteoporotic group (p>0.1). Serum Ca+2 and ALP values in postmenopausal group were higher than postmenopausal and premenopausal normal groups had. There was no significant relationship in Pi levels of blood and urine (p>0.05). We observed a prominent decrease in Dpd/Cre values of the patients treated with alendronate for 4 months compared to the values before treatment and after treatment (p<0.05). In conclusion, this study showed that deoxypyridinoline which is a degradation product of collagen in the urine is a good marker in the diagnosis and follow up of osteoporosis.
