Amaç: Alveoler kemik ögmentasyonu için yönlendirilmiş kemik rejenerasyonu, alveoler kret ayırma ve otojen kemik ile greftleme gibi birçok teknik ortaya konmuştur. Bu teknikler, kemik rejenerasyonu ile ilişkili osteoindüktif, osteokondüktif içeriği barındıran özelliklere sahiptir. Alveoler kret ayırma, tüm bu yöntemler arasında en güvenilir sonuç veren teknik olarak öne çıkmaktadır. Ayrıca diğer yöntemlerle karşılaştırıldığında, aynı seansta implant uygulanmasıyla zaman tasarrufu sağlamakta ve maliyeti düşmektedir. Çalışmamızın amacı, alveoler kret ayırma yönteminin ardından uygulanan dental implantların çevresinde meydana gelen kemik rezorpsiyonunu retrospektif olarak değerlendirmektir. Gereç ve Yöntemler: Bu retrospektif çalışmada, atrofik mandibulaya sahip 21 hastaya alveoler kret ayırma yönteminin ardından aynı seansta uygulanan 29 adet dental implant incelendi. Mevcut kemik kalınlığı 3-5 mm aralığında olan çene kemiklerine, alveoler kret ayırma yönteminin ardından implantlar yerleştirilip boşluklar sığır kaynaklı greft materyali ile dolduruldu. Altı aylık iyileşme sürecinin ardından alınan radyograflar ile implant çevresindeki kemikte meydana gelen kemik rezorpsiyonu varlığı ve miktarı değerlendirildi. Aynı zamanda implantların başarı oranı da değerlendirildi. Bulgular: Yaş ortalaması 47,7±10,8 yıl olan 21 hastaya uygulanan 29 implantın 6. ay sonunda sağkalım oranı %96,6 olarak bulundu. Hastaların %23,8 (n=5)'inde intraoperatif komplikasyon gelişti, %4,8 (n=1)'inde ise postoperatif komplikasyon gözlendi. İmplant çevresinde meydana gelen kemik rezorpsiyon miktarı ortalama 0,2 mm olarak bulundu. Sonuç: Alveoler kret ayırma yöntemi, mevcut kemik kalınlığının yetersiz olduğu durumlarda kemik genişliğini artırmakta ve aynı seans implant yerleşimine olanak sağlamaktadır. Diğer yöntemlere göre zaman kaybını önleyen bu yöntem, implant sağkalımı açısından yüksek başarı oranı göstermektedir.

Objective: A lot of techniques including guided bone regeneration, alveolar ridge split, and autologous bone grafting are used for alveolar bone augmentation. These methods contain osteoinductive and osteoconductive properties that are related to bone regeneration. Alveolar ridge split is the most reliable method among these techniques. It also prevents waste of time and decreases the cost via performing implant immediately after the split procedure. This study aims to determine retrospectively the resorption around the implants that were performed after the alveolar ridge split. Material and Methods: In this retrospective study analyzes 21 patients who were performed alveolar ridge split and 29 dental implants immediately. The jaws that have 3-5 mm residual bone were done alveolar ridge split and implants were placed. Then spaces were filled with bovine bone graft. After the healing period for 6 months, radiographs were taken and existing and amount of resorption were determined around implants. Implant survival rate also assessed. Results: The 29 implants that were performed to 21 patients -mean age 47.7±10.8- had a 96,6% survival rate. 23.8% of patients (n=5) had intraoperative complications. 4.8% (n=1) of them had a postoperative complication. The mean bone resorption around implants was 0.2 mm. Conclusion: Alveolar ridge split method is performed when the residual bone volume is insufficient, and it provides implant placement immediately. This technique prevents waste of time and it increases the implant survival rate.
