Değerli Meslektaşlarım,
Türkiye Klinikleri'nin 'Kornea Hastalıklarında Ön Segment Görüntüleme ve Ayırıcı Tanıda Kullanımı' konulu kitabının hazırlanmasında görev almaktan onur duymaktayım.
Günümüzde teknolojideki hızlı gelişme ile birlikte oftalmolojide kullanılan görüntüleme cihazları daha hızlı ve daha yüksek çözünürlükte görüntüleme yapma imkânı sunmaktadır. Bu durum çeşitli kornea hastalıklarının tanı ve ayırıcı tanısında farklı görüntüleme sistemlerinin tek başına veya geleneksel bir yöntem olan biyomikroskopi ile birlikte yaygın bir şekilde kullanılmasına katkı sağlamıştır.
Kornea ve ön segmentin değerlendirilmesinde sık olarak kullanılan cihazlar, kornea tomografi ve topografisi, ultrason biyomikroskopi (UBM), in vivo konfokal mikroskopi (IVKM), speküler mikroskopi, optik koherens tomografi (OKT) ve anjiyografi (OKTA) ile ̇ dansitometrik ve biyomekanik ölçüm sistemleridir. Ayrıca son yıllarda farklı görüntüleme cihazları ile elde edilen verilerin bir arada yorumlandığı multi-modal platformlar ile kornea ve ön segment hastalıklarının tanısında özgüllük ve duyarlılığı yükseltmek hedeflenmektedir.
Kornea enfeksiyonları, distrofi ve dejenerasyonları ile yüzey tümörleri ve kök hücre yetmezliği gibi oküler yüzey hastalıklarının değerlendirilmesinde biyomikroskopik bulguların, mikrobiyolojik, patolojik ve görüntüleme sistemlerinden elde edilen veriler ile birlikte yorumlanması in vivo tanı ve ayırıcı tanı yapmayı kolaylaştırmaktadır. Refraktif cerrahi başta olmak üzere çeşitli kornea cerrahilerinde preoperatif görüntüleme yapmak ise olmazsa olmazdır. Ayrıca cerrahi sırasında OKT kullanımının özellikle lameller kornea cerrahisinde başarıyı artırdığı da çeşitli çalışmalarda gösterilmiştir.
Türkiye Klinikleri'nin bu kitabında; korneanın ektazik hastalıkları, enfeksiyon, distrofi ve dejenerasyonları, tümörleri, limbal kök hücre yetmezliği gibi oküler yüzey hastalıklarında ve kornea cerrahisinde görüntüleme yöntemlerinin kullanımı ve sonuçların değerlendirilmesi ile ilgili bilgiler verilmiştir. Okuyucular için güncel ve faydalı bilgiler içeren konuları kendi deneyimleri ve olgu görüntüleri ile zenginleştirerek derleyen tüm yazarlara teşekkürlerimi sunarım.

I am very honored to take part in the preparation of the book about 'Anterior Segment Imaging and Differential Diagnosis Use of Corneal Diseases' of Türkiye Klinikleri. Today, with the rapid development in technology, imaging devices used in ophthalmology offer faster and higher resolution imaging. This situation has contributed to the widespread use of different imaging systems either alone or in conjunction with biomicroscopy, which is a traditional method, in the diagnosis and differential diagnosis of various corneal diseases.
Commonly used devices in the evaluation of the cornea and anterior segment are corneal tomography and topography, ultrasound biomicroscopy (UBM), in vivo confocal microscopy (IVKM), specular microscopy, optical coherence tomography (OCT) and angiography (OCTA), and densitometric and biomechanical measurement systems. In addition, research in recent years has focused on increasing the specificity and sensitivity in the diagnosis of cornea and anterior segment diseases with multi-modal platforms, where data obtained with different imaging devices are interpreted collectively.
Interpretation of biomicroscopic findings with data obtained from microbiological, pathological and imaging systems in the evaluation of ocular surface diseases such as corneal infections, dystrophy and degeneration, surface tumors and stem cell deficiency, facilitates in vivo diagnosis and differential diagnosis. Preoperative imaging is indispensable in various corneal surgeries, especially refractive surgery. In addition, it has been shown in various studies that the use of OCT during surgery increases the success, especially in lamellar corneal surgery.
This book of Türkiye Klinikleri covers information about imaging methods and evaluation of the results for the ectatic diseases of the cornea; infection, dystrophies and degenerations; tumors, ocular surface diseases such as limbal cell deficiency and corneal surgery. I would like to express my gratitude to all the authors who compiled the topics that contain current and useful information for the readers, enriching them with their own experiences and case images.
