ÖZET
İnferior miyokard infarktüsü (MI) geçirmiş, klinik olarak belirgin sağ kalp yetmezliği bulunmayan olgularda, sağ ventrikül (SV) disfonksiyonunun, hepatojuguler reflü (HJR) ile gösterilebilirliğini araştırmak ve sağ taraf kalp basınçları ile uyumunu incelemek amacıyla; inferior MI geçirmiş, koroner anjiyografide sadece sağ koroner arter lezyonu bulunan ya da sirkumfleks(SK) arteri dominant olanlarda, sadece SK lezyonu olan tek damar hastası, 20 olgu çalışmaya alındı. Akut MI, klinik kalp yetmezliği (sol ventrikül ejeksiyon fraksiyonu (EF) <%40), korpulmonale bulunan olgular çalışmaya alınmadı. Ayrıca koronerleri normal bulunan 10 olguluk kontrol grubu oluşturuldu. Olgular 45 derece yatar pozisyona getirildikten sonra, karın çevresini çepeçevre saracak şekilde özel hazırlanmış manşonlu kılıf epigastrik seviyeden sarıldı. Manşon kılıf şişirilmeden önce ve 35 mmHgya kadar şişirildikten 1 dakika sonra, boyun venöz dolgunluğu incelenip, sağ kateterizasyon ile wedge, pulmoner arter (PA), sağ ventrikül (SV) ve sağ atriyum (SA) basınçları alındı. HJR, venöz dolgunluğu 4 cmden fazla olanlarda pozitif kabul edildi. Ayrıca olguların eko incelemeleri yapıldı. Hasta grubunda 20 olgudan 9da (%45), HJR pozitif bulundu. Kontrol grubunda pozitif HJRye rastlanmadı. Hasta grubunda; SA ortalama, SV diyastolik, PA ortalama ve wedge basıncı, test sonrası anlamlı artış gösterdi (sırasıyla p<0,05- p<0,01- p<0,05- p<0,05). Kontrol grubunda ise sözkonusu ölçümlerde bir artış gözlenmedi. HJR pozitif olgularda SA ortalama ve SV diyastolik basınçları test sonrası anlamlı artarken, negatif olanlarda anlamlı artış gözlenmedi. Hasta grubunda kontrol grubuna göre, HJR pozitif olanlarda negatif olanlara göre, SV ve SA büyüklüğü daha fazla bulundu. Kontrol grubunda triküspit yetmezliğine (TY) rastlanmazken, hasta grubunda 11 olguda TY saptandı (p<0,01). Klinik olarak belirgin sağ kalp yetmezliği bulunmayan inferior MIlı olgularda, HJRnin SV disfonksiyonunu göstermede basit, güvenilir bir yöntem olduğu sonucuna varıldı.

ABSTRACT
The aim of this study was to show the value of hepatojugular reflux (HJR) in determination of the right ventricular (RV) dysfunction and ıts correlation with the right heart pressures in patients with previous inferior myocardial infarction (MI) but no signs of apperent RV failure. The study consists of 20 patients having previous inferior MI with single vessel lession either in the right coronary artery or in the circumflex artery (if the circumflex artery is the dominant artery). We excluded the patients with acute MI, clinically heart failure (ejection fraction (EF) <40%), or corpulmonale. The control group concluded 10 patients with normal coronary angiograms. Patients were situated in 45 degrees supine position. A specially designed coupling was wrapped around the abdomen at the epigastric level. We determined the level of the jugular venous distention and measured right atrial (RA), right ventricular (RV), pulmonary artery (PA) and wedge pressures, in the resting position and 1 minute following the inflation of the coupling at 35 mmHg. Venous distention exceeding 4 cm was regarded as positive test. Echocardiographic examination were also performed to determine the RA, RV dimensions and the presence of the tricuspid insufficiency. In 9 of the 20 study group patients (45%), HJR was positive, whereas negative in all of the control patients. Significant increaments were observed in the RA mean, RV diastolic, PA mean and wedge pressures in the study group comparing the test results with the resting measurements (p<0.05, p<0.01, p<0.05, p< 0.05, respectively). In the control group no difference occured. In a subgroup of the study group patients having positive HJR, increase of the RA mean and RV diastolic pressures were significant but not in the patients with negative HJR. Comparing the study group with the control group and HJR positive group with negative group, the RV and RA dimensions were larger in the study group and also HJR positivegroup but in 11 of the study group patients (p<0.01). In patients with inferior MI without apperent right ventricular failure, HJR was established to be an easy and safe method to determine the RV dysfunction.
