ÖZET
Amaç: Çalışmanın amacı, timpanik membran tipi kayıt elektrodu ile yapılan elektrokokleografi (TM ECoG) testinin endolenfatik hidropsu göstermedeki başarısını incelemektir.Gereç ve Yöntem: 1995 AAO-HNSnin yayınladığı kriterlere göre mutlak ve olası Meniere olgusu olarak kabul edilen toplam 36 kişiden oluşan ayrı iki çalışma grubu, bunlardan tek taraflı olanların asemptomatik kulaklarından iki ayrı grup, 12 sağlıklı bireyden de kontrol grubu oluşturularak TM ECoG kayıtları incelenmiştir. Testlerde kayıt elektrodu olarak kulak zarına temas ettirilen hidrojel elektrodlar kullanılmış olup, 80-100 dB nHL seviyesinde toplam 512 klik uyarıya karşılık elde edilen potansiyellerin avarajlanması ile elde edilen cevaplar 10-3000 Hzlik band-pass filtreden geçirilmiştir. Endolenfatik hidropsun araştırılmasında test kriteri olarak, alternan klik ile elde edilmiş cevaptaki sumasyon potansiyeli/aksiyon potansiyeli (SP/AP) oranı ile kondensasyon ve rarefaksiyon klik ile elde edilen aksiyon potansiyellerinin arasındaki latans farkları alınmıştır.Bulgular: Kontrol grubundan elde edilen bilgiler doğrultusunda SP/AP oranı için normalin üst sınırı, ±2 standart deviasyon gözönüne alınarak 0.32 (0.22±0.05), latans farkı için ise 0.38 ms (0.24±0.07) olarak belirlenmiş ve bu değerleri geçen kayıtlar patolojik kabul edilmiştir. Ortalama SP/AP oranı ve AP latans farkı mutlak Meniere grubunda sırasıyla 0.34±0.16 ve 0.19msn±0.13, muhtemel Meniere grubunda ise 0.27±0.12 ve 0.19ms±0.24 olarak ölçülmüştür. SP/AP oranı yönünden mutlak Meniere grubu ile kontrol grubu karşılaştırılmış ve aralarında anlamlı fark izlenmiştir (p<0.001).Sonuç: Sadece SP/AP oranına bakarak mutlak Meniere olgularının %53ünde patolojik TM ECoG bulgusu saptanırken, latans farkı kriterinin eklenmesinin ilave bir katkısı olmamıştır. Tek taraflı mutlak Meniere grubunun asemptomatik kulaklarının %33ünde patolojik ECoG bulgusu görülmüştür. Endolenfatik hidrops şüphesi olan olgularda testin pozitif olması tanıyı büyük oranda desteklemektedir. Ancak test negatif bulunursa endolenfatik hidropsu dışlamak çok doğru gözükmemektedir.

ABSTRACT
Objective: The purpose of the study is to investigate the diagnostic value of the tympanic membrane electrocochleography (TM ECoG) in patients with endolymphatic hydrops. Materials and Methods: TM ECoG recordings were obtained from the study group of 36 subjects with definite and probable Menieres disease based on the criteria recommended by AAO-HNS (1995), two groups of asymptomatic ears with unilateral disease and a control group of 12 healthy volunteers with no balance and hearing disorder. For the recordings, 512 click stimuli were delivered at intensity levels ranging from 80 to 100 dB nHL, and responses, picked up by a tympanic membrane type hydrogel electrode were averaged and filtered through 10-3000 Hz. Two parameters were investigated in diagnosis of endolymphatic hydrops, one being the summation potential (SP)-action potential (AP) amplitude ratio in response to alternating click stimulus and second latency difference between the APs, each obtained as response to rarefaction and condensation clicks.Results: The upper limits of SP/AP ratio and latency difference based on the control group data and its ±2 standard deviation were 0.32 (0.22±0.05) and 0.38 ms (0.24±0.07). Measures those exceeding these limits were considered abnormal. Mean SP/AP ratio and AP latency difference were 0.34±0.16, 0.19ms±0.13, and 0.27±0.12, 0.19 ms±0.24 in the definite Menieres group and probable Menieres group respectively. A significant difference was observed between the definite Menieres group and the control group with regard to SP/AP ratio (p<0.001). Conclusion: Only 53% of the definite Menieres group showed abnormal ECoG when SP/AP ratio was taken as a sole criterion. Adding the latency difference as a additional criterion did not enhance diagnostic utility of the test. Asymptomatic ears of unilateral definite group did have abnormal ECoG result in 33% of the group. As a result, while a positive test result seems to be an evidence of endolyphatic hydrops, a negative result can not rule out the hydrops.
