Evaluation of the Relationship Between Childhood Maltreatments and Cancer Using the Case-Control Research Method

ÖZET
Amaç: Çocukluk çağındaki ruhsal travmalar, çocukların büyüme ve gelişimini olumsuz etkileyen, görülme sıklığı yüksek önemli sosyal çevre faktörlerinden biridir. Bu araştırmanın amacı, çocukluk çağı örselenmeleri ile kanser arasındaki ilişkinin incelenmesidir. Gereç ve Yöntemler: Olgu-kontrol araştırma yöntemiyle yürütülen bu çalışmanın örneklemini hastanede tedavi edilen 63 erişkin kanser hastası ve hastaneye başvurmuş benzer cinsiyet, öğrenim ve ekonomik duruma sahip kanser tanısı almamış 89 erişkin oluşturmaktadır. Veri toplamada sosyodemografik bilgi formu ve Çocukluk Dönemi Örselenme Yaşantıları Ölçeği Kısa Formu kullanılmıştır. Normal dağılım gösteren verinin analizinde t-testi, varyans analizi ve etki büyüklüğünün bir ölçüsü olan tahmini rölatif risk [odds ratio (OR)] kullanılmıştır. Bulgular: Katılımcıların yaklaşık %51'i kadın, %49'u erkektir. Cinsiyet, gelir durumu, öğrenim durumu ve yaş dağılımı olgu-kontrol gruplarında benzerdir (p>0,05). Duygusal istismar, duygusal ihmal ve fiziksel ihmal ile kanser arasında anlamlı ilişki olduğu (sırasıyla OR=2,48, 1,86, 3,42), belirtilen 3 örselenme türünün kanser olma riskini artırdığı saptanmıştır. Fiziksel istismar ve cinsel istismar puan ortalamaları olgu-kontrol gruplarına göre anlamlı farklılık göstermemiştir (p>0,05). Kanser hastalarının psikolojik destek alma sıklığı ise %12,7'dir. Sonuç: Çocukluk dönemi örselenmelerinin önlenmesi için birey odaklı ve topluma dayalı çalışmaların artırılmasının, erken uyarı sistemlerinin ve kanser tedavisinde travmaya duyarlı tedavi yaklaşımlarının geliştirilmesinin yararlı olacağı düşünülmektedir.

ABSTRACT
Objective: Childhood mental traumas are one of the most frequent and an important social environment factors that negatively affect the growth and development of children. The aim of this study is to examine the relation between childhood maltreatments and cancer. Material and Methods: In this case-control study, the sample of the study consists of 63 adult patients with cancer were treated at the hospital and 89 adults who were admitted to the same hospital with similar gender, education, and economic status and were not diagnosed with cancer. Socio-demographic information form and Childhood Trauma Questionnaire Short Form were used in data collection. In the analysis of normally distributed data, t-test, analysis of variance, and odds ratio (OR), which is a measure of effect size, were used. Results: Approximately 51% of the participants were female and 49% were male. Gender, income, education and age distribution were similar in the case-control groups (p>0.05). Emotional abuse, emotional neglect, and physical neglect were associated with having cancer (respectively OR=2.48, 1.86, 3.42), all mentioned maltreatment types were increased odds of having cancer. Physical abuse and sexual abuse mean scores did not differ significantly between the case-control groups (p>0.05). The rate of receiving psychological support was 12.7%. Conclusion: The results of the study showed that the importance of individual and community-based studies to prevent childhood maltreatment, development of early warning systems, and the need for trauma-sensitive approaches in the cancer treatment process.

1. TERMS OF USE

1.1. 4.2. People benefiting from the services provided by "Turkiye Klinikleri" and using the website can use the "SITE" only according to the law and only for personal reasons. "Force majeure" term will be regarded as outside of the concerned party's reasonable control and any situation that "Turkiye Klinikleri" cannot prevent even though it shows due diligence. Also, force majeure situations include but not limited to natural disasters, rebellion, war, strike, communication problems, infrastructure and internet failure, power cut and bad weather conditions.

8. LAW AND AUTHORISATION TO FOLLOW

Turkish Law will be applied in practicing, interpreting the hereby "Terms of Use" and managing the emerging legal relationships within this "Terms of Use" in case of finding element of foreignness, except for the rules of Turkish conflict of laws. Ankara Courts and Enforcement Offices are entitled in any controversy happened or may happen due to hereby contract.

9. CLOSING AND AGREEMENT

Hereby "Terms of Use" come into force when announced in the "SITE" by "Turkiye Klinikleri". The users are regarded to agree to hereby contract terms by using the "SITE". "Turkiye Klinikleri" may change the contract terms and the changes will be come into force by specifying the version number and the date of change on time it is published in the "SITE".
