Son yıllarda, tüm dünyada olduğu gibi Türkiye'de de toplumun akupunktura ilgisi artmıştır; doğal olarak hekimlerin de toplumun bu ilgisine karşılık verebilmesi ve sorularını yanıtlayabilmesi önemlidir. Bu sistematik derlemenin amacı, 1. basamakta akupunktur uygulanarak yapılmış çalışmaları inceleyerek, 1. basamak hekimlerine literatür özeti sunabilmektir. Sistematik derleme türündeki bu çalışmada, 'PubMed' veri tabanı 'primary care and acupuncture', 'birinci basamak ve akupunktur' anahtar kelimeleri kullanılarak taranmıştır. Çalışmaya son 10 yılda yapılmış, ücretsiz tam metnine ulaşılabilen, yayın dili İngilizce veya Türkçe olan klinik çalışmalar, metaanalizler, randomize kontrollü çalışmalar ile sistematik derlemeler alınmıştır. PubMed veri tabanında, 1.3.2021 tarihinde anahtar kelimeler ve dâhil etme kriterine göre yapılan taramada 220 tane makale tespit edildi. Bu makalelerin %43,18'i (n=95) sistematik derleme ve metaanaliz, %56,81'i (n=125) ise randomize kontrollü çalışma grubunda yer almaktadır. Randomize kontrollü çalışmaların 14'ü, 1. basamakta yapılmayan fakat 1. basamağı ilgilendiren konularda yapılmış çalışmalar idi. Birinci basamakta yapılan sadece 8 çalışma tespit edildi. Bu 8 makalede, depresyon ve anksiyete, irritabl bağırsak sendromu, menopoz semptomları, kronik bel ağrısı ve tıbben açıklanamayan belirtiler için akupunkturun etkinliği incelenmişti. Bu çalışma, akupunkturun 1. basamak hastalıklarında uygulamaları ile ilgili kanıtların henüz yetersiz olduğunu göstermektedir. Özellikle 1. basamak hekimlerinin ve Türkiye'de akupunktur uygulaması yapan hekimlerin yapacağı geniş tabanlı, kanıt düzeyi yüksek çalışmalara ihtiyaç vardır.

In recent years, society's interest in acupuncture has increased in Turkey as well as in the rest of the world, and it is naturally important for physicians to respond to this interest of the society and answer their questions. The aim of this systematic review is to present a literature summary to primary care physicians by examining the studies conducted using acupuncture in primary care. In this systematic review, the 'PubMed' database was searched using the keywords 'primary care and acupuncture', 'birinci basamak ve akupunktur'. The study included clinical studies, meta-analyses, randomized controlled trials, and systematic reviews that were conducted in the last 10 years, whose full text is available for free and whose publication language is English or Turkish. On 1.3.2021, 220 articles were found in the PubMed database, according to keywords and inclusion criteria. Systematic review and meta-analysis were 43.18% (n=95) of these articles while 56.81% (n=125) were randomized controlled trial. Fourteen of the randomized controlled trials were not conducted in primary care, but were about issues related to primary care. Only 8 studies were identified that were done in primary care. In these 8 articles the efficacy of acupuncture for depression and anxiety, irritable bowel syndrome, menopausal symptoms, chronic low back pain, and medically unexplained physical symptoms were analyzed. This study shows that the evidence regarding the applications of acupuncture in primary care diseases is still insufficient. There is a need for large scaled and highly leveled in evidence studies especially made by primary care physicians and physicians practicing acupuncture in Turkey.
