Behçet Hastalığı Dışındaki Üveitlerde Göz Bulguları
EYE SIGNS IN UVEITIS EXCEPT FOR BEHCET'S DISEASE

ÖZET
Amaç: Üveit tanısı alan olguların cinsiyet, yaş, takip süresi, atak sayısı ve eşlik eden göz muayene bulgularını retrospektif olarak incelemek. Gereç ve Yöntemler: 1996-2006 yılları arasında SB Ankara Ulucanlar Göz Eğitim ve Araştırma Hastanesinde üveit tanısı alan ve üvea kliniğinde takip altına alınan 62 hasta retrospektif olarak incelendi. Behçet hastalığı tanısı alanlar çalışma dışında bırakıldı. Olguların görme değerleri, göz içi basıncı değerleri, biyomikroskopi bulguları ve pupil dilatasyonu sonrası göz dibi incelemesini içeren tüm göz muayene bulguları değerlendirildi. Bulgular: Takip edilen olguların 34'ü erkek (%54.83), 28'i kadındı (%45.16). Erkek hastaların yaş ortalaması 34.66 yıl (13-70 yıl), kadın hastaların yaş ortalaması ise 42.03 yıl (16-66 yıl) idi. Ortalama tanı süreleri 2.82 yıl (1-10 yıl), takip süreleri ise ortalama 2.81 yıl (1-10 yıl) idi. Toplam 62 hastanın 87 gözü tutulmuştu. Otuz yedi hastada tek taraflı, 25 hastada iki taraflı göz tutulumu mevcuttu. Üveit atak sıklığı 10 yıllık takip süresinde ortalama 1.33 idi (1-5 atak). Toplam 87 gözün 43'ünde (%49.42) ön üveit, 2'sinde (%2.29) pars planit, 33'ünde (%37.93) arka üveit, 9'unda (%10.34) panüveit mevcuttu. Ön segment komplikasyonu olarak 32 gözde (%36.78) katarakt, 11 gözde (%12.64) sekel keratik presipitat, 10 gözde (%11.49) arka sineşi, 4 gözde (%4.59) arka kapsül opasifikasyonu ve 1 gözde (%1.14) bant keratopati belirlendi. Arka segment komplikasyonu olarak ise 20 gözde (%22.98) vitreus kondansasyonu, 8 gözde (%9.19) kistoid makula ödemi, 3 gözde (%3.44) ven dal tıkanıklığı, 2 gözde (%2.29) optik atrofi ve 2 gözde (%.2.29) korioretinit odağı saptandı. Etiyolojik yönden yapılan değerlendirmede 3 olguda (%4.83) juvenil romatoid artrit, 2 olguda (%3.22) ankilozan spondilit, 1 olguda (%1.61) herpetik keratoüveit, 1 olguda (%1.61) Harada hastalığı ve 1 olguda (%1.61) sarkoidoz tespit edildi. Elli dört olguda ise (%87.09) herhangi bir etiyolojik neden tespit edilemedi ve bu hastalar idiopatik grup olarak değerlendirildi. Sonuç: Behçet hastalığı olmayan üveitli hastalarda etiyolojik olarak en büyük grubu idiopatik grup teşkil etmekte idi. İkinci sırada ise romatizmal hastalıklar yer almaktadır. En sık görülen ön segment komplikasyonu olarak katarakt, arka segment komplikasyonu olarak kistoid makula ödemi gözlenmektedir. Ön üveit ve arka üveit insidansları ise birbirine yakın oranlarda gözlemlenmektedir. En önemli görme kaybı arka segment iltihabı sonucu ortaya çıkmaktadır.

ABSTRACT
Objective: To investigate the sexuality, average age, mean follow up period, mean frequency of uveitis and accompanying eye examination findings of cases that were diagnosed as uveitis retrospectively. Material and Methods: From 1996 to the 2006, 62 patients with uveitis in SB Ankara Ulucanlar Eye Hospital Uvea Clinic were included in this study retrospectively. Cases diagnosed as Behçet's disease were extracted from the study. A complete ophthalmological examination was evaluated including visual acuity, applanation tonometry, biomicroscobic examination findings and dilated examination of the posterior segment. Results: 28 women (45.16%) and 34 men (54.83%) were available for follow up evaluation. The average age of men was 34.66 years (13-70 years). The average age of women was 42.03 years (16-66 years). Mean diagnosis time was 2.82 year (1-10 years). Mean follow up period was 2.81 years (1-10 years). Eighty-seven eyes of 62 patients were involved. Thirty-seven patients had unilateral, and 25 patients had bilateral involvement. Mean number of uveitis attack in 10 year follow-up period was 1.33 (1-5 attack). At the onset of the follow up, anterior uveitis, intermediate uveitis, posterior uveitis and panuveitis were determined in 43 (49.42%), 2 (2.29%), 33 (37.93%) and 9 (10.34%) of 70 eyes respectively. Cataract in 32 eyes (36.78%), sequel keratic precipitates in 11 eyes (12.64%), posterior synechiae in 10 eyes (11.49%), posterior capsular opacification in 4 eyes (4.59%), and band keratopathy in 1 eye (1.14%) were found as anterior segment findings. Vitreous condensation in 20 eyes (22.98%), cystoid macular edema in 8 eyes (9.19%), retinal vein branch occlusion in 3 eyes (3.44%), optic atrophy in 2 eyes (2.29%) and chorioretinitis area in 2 eyes (2.29%) were found as posterior segment findings. Juvenile rheumatoid arthritis in 3 cases (4.83%), ankylosing spondylitis in 2 cases (3.22%), herpetic keratouveitis in one case (1.61%), Haradas disease in 1 case (1.61%), and sarcoidosis in 1 case (1.61%) were detected as etiologic causes. No causes were found in 54 cases (87.09%) and these were evaluated as idiopathic group. Conclusion: Idiopathic group was the biggest group as etiologic cause in uveitis except for Behçets disease. Romatoid disease was the second group. Cataract was the most common anterior segment complication. Cystoid macular edema was the most common posterior segment complication. Anterior segment findings and posterior segment findings were observed in nearly equal rates in our study. Posterior segment inflammation was the most serious cause of visual loss.
