Solunum yolu hastalıklarının tedavisinde, inhaler tedavi kullanımının başlamasıyla birlikte bu tedavi şekline ilişkin sorunlar da ortaya çıkmaya başlamıştır. Tedavi uyumsuzluğu, ilaç tedavilerinin kalitesi ve maliyetinde merkezî bir sorun olarak yer almaktadır. Astım hastalarında; kötü ilaç uyumu, hastalığı kontrol edememe, sık alevlenmeler ve artan mortalite ile ilişkilidir. Astım hastalarındaki tedavi uyumsuzluğu, diğer birçok kronik hastalıktan daha yüksek olduğu bilinmektedir. Bu muhtemelen inhaler uygulamasının algılanan karmaşıklığından ve bu ilaç verme yoluna olası bir güven eksikliğinden kaynaklanmaktadır. İnhaler ilaç kullanımında doğru teknik, tedavinin etkinliği için tedavi uyumu kadar önemli bir diğer faktördür. Yanlış inhaler teknik ile zayıf hastalık kontrolü, artmış alevlenmeler ve artmış hastalık maliyetleri arasında çok net bir ilişki vardır. İnhaler tedavilerdeki gelişmelere rağmen inhaler teknikteki saptanan hatalar hâlâ istenmeyecek ölçüde yüksektir. Bu nedenle bireysel hasta ihtiyaçlarını karşılamak için en uygun cihazı seçmek, klinik uygulamada önemli bir husustur. Bu konuyla ilgili çözüm önerilerine yönelik yaklaşımlar uygulamaya konulmalıdır. Gerçek hayatta inhaler tedavilere uyum oranının düşük ve inhaler teknik hata oranlarının hâlâ yüksek olduğu aşikârdır. İyileştirmek için girişimlerde bulunulmalıdır. Ancak daha alınacak uzun bir yol olduğu da açıktır.

With the beginning of the use of inhaler therapy in the treatment of respiratory diseases, problems related to this form of treatment have also begun to emerge. Treatment non-compliance is a central issue in the quality and economics of drug treatments. In asthma patients, poor medication adherence is associated with inability to control the disease, frequent exacerbations, and increased mortality. It is known that treatment non-adherence in asthma patients is higher than many other chronic diseases. This is likely due to the perceived complexity of inhaler administration and a possible lack of confidence in this route of administration. Correct technique in inhaler drug use is another factor as important as treatment compliance for the effectiveness of the treatment. There is a very clear correlation between the wrong inhaler technique and poor disease control, increased exacerbations and increased disease costs. Despite the advances in inhaler treatments, the errors detected in inhaler technique are still undesirably high. Therefore, choosing the most appropriate device to meet individual patient needs is an important consideration in clinical practice. Necessary measures regarding this issue should be implemented as soon as possible. It is a fact that in real life, the rate of adherence with inhaler therapies is low and the inhaler technical error rates are still high. Attempts should be made to improve it. However, it is clear that there is still a long way to go.
