Akut Miyokard İnfarktüsünde ²Excimer² Lazer Koroner Anjiyoplasti Uygulamaları

ÖZET
Primer anjiyoplasti ile olguların önemli bir kısmında TIMI III akım sağlamak mümkün değildir. Bu çalışmada akut miyokard infarktüsünde perkütan ?excimer lazer koroner anjiyoplasti uygulamalarımızın kısa dönem sonuçlarını inceledik.Akut miyokard infarktüsü tedavisi amacıyla kateter laboratuvarına alınan ve Excimer Lazer Koroner Anjiyoplasti (ELKA) uyguladıgımız 18 hasta çalışmaya alındı. İkisi kadın,16sı erkek olan hastaların yaş ortalaması 56.6 ± 12 yıl idi.İşlem öncesi 10 olguda TIMI-O, 5 olguda TIMI-I ve 3 olguda TIMI-II akım vardı. Lezyonların 12si (%67) tip C (total trombotik oklüzyon), 6sı (%33) tipB2 (kompleks trombotik lezyon ) özellik gösteriyordu. İşlem öncesi ortalama yüzde darlık %97.89 ± 5.06 idi. Vakaların 3ünde 1.4 mm çapında, 13 ünde 1.7 mm çapında, 2sinde önce 1.4 mm ardından 1.7 mm çapında lazer kateter kullanıldı. Kateterlerin tümü konsantrikti. Lazer emisyon akışı 45mj/m2, tekrarlama hızı 25 atım / sn idi. Tüm olgularda lezyon kateter ile geçildi. Ortalama lazer atım sayısı 808±384 idi. Lazer anjiyoplasti sonrası bir olguda tip C (%5.5) disseksiyon gelişti. İşlem sonrası distal akım bu olgu hariç (bunda TIMI-II) diğerlerinde TIMI-III idi. Perforasyon gelişmedi. Lazer anjiyoplasti sonrası, 90. dakikada tüm olgularda %70in üzerinde ST rezolüsyonu sağlandı. İşlem öncesi hiçbir olguda TIMI-III akım saptanmazken, lazer anjiyoplasti sonrası 17 olguda (%95) TIMI III akım sağlandı. ELCA öncesi ortalama Minimal Lümen Çapı (MLÇ) 0.11 ± 0.2 mmden, 1.64 ± 0.2 mmye (p=0.0001) yükseldi. Stent sonrası MLÇ 3.35 ± 0.4mm idi. Lazer başarısı %95 idi. Lazer sonrası hiçbir olguda ek balon anjiyoplasti yapılmadı. Tüm olgulara stent implante edildi. İşlem başarısı %100 idi.Sonuç olarak, akut miyokard infarktüsünün girişimsel tedavisinde lazer anjiyoplasti etkin ve güvenli bir tedavi seçeneğidir.

It is not possible to achieve TIMI III flow with primary angioplasty in the majority of the cases. In this study we reported the short-term results of percutanous excimer laser angioplasty in acute myocardial infarction.18 patients who were taken to the cath-lab with the purpose of primary percutaneous intervention and underwent Excimer Laser Coronary Angioplasty (ELCA)were enrolled to the study. Of these patients, 2 were female and 16 were male with an average age of 56.6 ± 12 yrs. TIMI 0, TIMI I and TIMI II flows were observed in 10, 5 and 3 cases respectively prior to the procedure. Twelve of the lesions (67%) were type C (total thrombotic occlusion) and 6 (33%) were type B2 (complex thrombotic lesion). Average percent stenosis prior to the procedure was 97.98 ± 5.06%. Prior to the procedure 100% of the cases had thrombosis. In 3 of the cases 1.4 mm, in 13 of the cases 1.7 mm and in 2 of the cases 1.4 mm and 1.7 mm laser catheters were used successively. All catheters were concentric type. The fluence of laser emission was 45 mj/m2, the repetition rate was 25 pulses per second. The lesion was crosssed with a laser catheter in all cases. Average laser pulses were 808 ± 384. Dissection type C (5.5%) was developed in only one case. Except this case, in all other cases distal flow was TIMI-III. No perforation was observed. Following the procedure, in the first 90 minutes, ST resolution of more than 70% was achieved in all cases. Post-ELCA, TIMI-III flow was achieved in 95% (17/18). Before ELCA, average MLD was 0,11 ± 0.2 mm and after ELCA, MLD was 1.64 ± 0.2 mm (p=0.0001). After stent placement, it was 3.35 ± 0.4 mm. The success of the laser was 95%. Adjuvant PTCA was not performed in none of the cases after laser application. Stent implantion was performed in all the cases. The procedural success rate was 100%. In conclusion, in interventional treatment of acute myocardial infarction, ELCA is an effective and reliable treatment option.
