Accuracy Of Intraoperative Frozen Section In Diagnosis Of Ovarian Tumors: The Role Of Pathologist: Experience Of Dokuz Eylul University Hospital

ÖZET
Amaç: Frozen kesit incelemesi over kaynaklı kitlelerin natürünü tespit etmek amacıyla intraoperatif olarak uygulanan önemli bir testtir. Over neoplazmalarının başarılı cerrahi tedavisi kitlelerin benign, borderline ve malign olarak doğru sınıflandırılması ile mümkün olmaktadır. Bu retrospektif çalışmanın amacı over neoplazmalarının frozen kesit incelemesinin doğruluk oranı üzerine jinekolojik patoloji alanında uzman olan ve olmayan patologların etkisini ortaya koymaktır. Gereç ve Yöntemler: Primer olarak cerrahi ile tedavi edilen over kaynaklı neoplazisi olan ve frozen kesit incelemesi yapılan 203 hastanın sonuçları retrospektif olarak değerlendirildi. Konusunda uzman (grup 1) olan ve olmayan (grup 2) patologların frozen kesit sonuçları, parafin kesit sonu kesin tanı sonuçları ile karşılaştırıldı. Bulgular: Altı olguda (%2.9) incelenen doku over kaynaklı değildi. Yedi olguda (%3.4) frozen ile bir sonuç alınamadı. Birinci grup patologların frozen incelemesinin sensitivitesi benign, borderline ve malign tümörler için sırasıyla %100, %100 ve %94.5 iken, spesifisiteleri sırasıyla %97.5, %99.3 ve %100 dür. Grup 2 patologların frozen kesit sensitiviteleri benign, borderline ve malign tümörler için sırası ile %95, % 40 ve % 75 iken, spesifisiteleri sırası ile %88.9, %95.4 ve % 93.3 tür. Frozen kesit incelemesinin genel doğruluk oranı % 92.4 tür. Sonuç: İntraoperatif frozen kesit incelemesinin over kaynaklı neoplazmaları olan hastaların cerrahi tedavisini yönlendirme konusunda yüksek doğruluk oranı vardır. Bu test jinekolojik patoloji alanında uzman patologlarca yapılırsa doğruluk oranları daha da artacaktır.
ABSTRACT
Objective: Frozen section is an important test to determine the nature of ovarian masses intraoperatively. The succesful surgical management of ovarian neoplasms depends on their correct categorization as benign, borderline or malignant. This retrospective study was undertaken to evaluate the accuracy of frozen section diagnosis of ovarian neoplasms and to determine the effects of experienced and unexperienced pathologists in gynecologic pathology field on intraoperative diagnosis. Material and Methods: İntraoperative frozen section diagnosis was retrospectively evaluated in 203 patients with suspected ovarian neoplasms who underwent surgery as primary line therapy. We compared the frozen section results of experienced( group 1) and unexperienced(group 2) pathologists with the final histopathologic diagnosis on paraffin section. Results: İn 6 cases (2.9%) tissues were other than ovaian, in 7 cases ( 3.4%) no opinion on frozen section was possible. Frozen section reports of group 1 pathologists had a sensitivity of 100%, 1005 and 94.5% for benign, borderline and malignant tumors. The corresponding specificities were 97.5%, 99.3% and 100% respectively. Frozen section reports of group 2 pathologists had sensitivities of 95%, 40% and 75% for benign, borderline and malignant tumors. The corresponding specificities were 88.9%, 95.4% and 93.3% respectively. The overall accuracy of frozen section diagnosis was 92.4%. Conclusion: İntraoperative frozen section has high accuracy to guide the surgical management of patient with ovarian neoplasms. The accuracy of this test is increased if this dignositic tool is performed by experienced pathologists with expertise in gynecologic pathology.
